Have you done anything to the tank recently?
Dose?
Water change?
Overfeed?
New fish?

Looks like it is a white cloudiness which would lead me to believe it is a bacteria bloom.

well i dosed chemiclean for cyano 2-3weeks ago. cleared up cyano, and no problems since.
no new fish, definitely don't over feed.
last 25% water change was 3 weeks ago right after the chemiclean dose.

It is most likely a bacteria bloom which is good in your case. I would guess from dosing the chemiclean some of the cyano has started to die off. The cyano was sucking up the nitrates and phosphate in your water so your water test reading were probably good but now the cyano is dieing off it is no longer sucking up the nitrates and phosphate so your bio-load has technically increased in the same way as if you were to add more fish. The bacteria bloom is because of this sudden increase in the bio-load. This is good because now hopefully the bacteria will naturally process the nitrates and phosphate and you will get less cyano. It should clear up in a few days.
